A 27-year-old man named Patrick Liam Coyle, from Wigan, tragically passed away shortly after appealing to medical professionals for assistance, as revealed in an inquest. Patrick was discovered lifeless at a residence in Accrington just six days after being discharged from a mental health facility. During a two-day inquest at County Hall in Preston, it was disclosed that Patrick had earnestly sought help in the days leading up to his demise.
After being discharged, Patrick visited the A&E department three days later, imploring to be readmitted on March 7. Despite his plea, he was deemed safe to be released as he claimed he had no intentions of self-harm. Regrettably, there was no contact with him after March 8, and he was found deceased two days later at the location.
The coroner, Kate Bisset, stated that Patrick had expressed a wish to return to inpatient care and had a history of mental health struggles. She mentioned that Patrick had struggled with alcohol addiction, which heightened the risks of impulsive actions and self-harm when intoxicated. Following his discharge, his mental state deteriorated, leading to excessive alcohol and medication consumption. Despite seeking help at the A&E, mental health workers assessed him as safe for release on March 7.
Support workers visited Patrick at home the next day, noting his denial of current suicidal thoughts. Plans were made for continued support, but Patrick was unresponsive after March 8 and was later discovered deceased on March 10. Due to his mental decline and intoxication, his intentions leading to the incident could not be determined.
